NPHS Band Apparel Order Form 2023
Use this form at the beginning of the year to order any necessary band apparel for the year. Send the form in with your students name and cash or check (payable to Panther Band Parents) to the band office. Every student will need the following at a minimum:
Concert Dress Shirt – This is a long sleeved black dress shirt. We order the same shirt for everybody to ensure uniformity, so please be sure to either purchase your own or ask for a loaner from the band. Please do not substitute your own shirt.
Dinkles Marching Shoes – These are black patent dress shoes that we use with the Marching Band uniforms. All students (except colorguard) need to have a pair of Dinkles to ensure uniformity, so be sure to either purchase your own or ask for a loaner pair from the band. Please do not substitute you own shoes.
Make Up Credit Procedure
Here are detailed instructions about how to receive credit for missing a performance. If you have any questions, contact Mr. Crystal as soon as possible to clarify.
Blank Field Trip Permission Slip
Submitted if your son/daughter will be attending a field trip off campus. Usually we collect one from every student at the beginning of each semester with all of our trips on it. You can use this in an emergency in case yours was misplaced.
Student Private Transportation Permission Slip
Submitted if your son/daughter needs to use private transportation to or from an event. This form must be turned in to Mr. Crystal at least 24 hours before the trip happens or the student will have to ride the bus to and from the event.
CVUSD Personal Vehicle Use Form
Submitted if you are going to drive a student (other than your own child) to or from an event.
Student Medication Authorization
Use this form if a student needs to take medication while the band is away on a trip. This form must be turned in with the medication in a Ziploc bag before we depart NPHS.
